Highbury Primary Health Care Team
Te Wakahuia Manawatū Trust was formed in 1998 and is based in Highbury, Palmerston North in response to strong calls from the local community to have their own health services provided by their own people.
The Trust provides mobile health services to some rural communities - these extend to other areas of Palmerston North/Manawatū, Tararua and Horowhenua regions.
The objective of the Trust is to ensure all health and social services provided meet the physical, spiritual, social and emotional needs of whanau Māori. Through mobilizing resources and skills, the Trust assists Maori communities to build the health and social capacity of individuals, whanau and the wider hapori, and support the achievement of Whanau Ora.
Te Wakahuia is a Primary Health Care Team (PHCT) that actively promotes shared leadership, continuous quality improvement in primary health care practice and service innovation.
The overall aim of the Highbury PHCT is to strive for excellence in the delivery of Whānau Ora Services to our Communities and improving access to community health and social services, co-ordination and continuity of care and reduce inequities for whanau through a collaborative approach to service delivery ensuring a more effective and efficient use of community health resources.
The Highbury PHCT comprises a number of highly skilled professionals from various disciplines, including nursing, community health workers, counsellors, social workers health promoters and kaiwhakaaraara, who work efficiently and effectively to assist whanau to realise their aspirations. Te Wakahuia is not a General Practice Service.
